A Survey Tool That Scales
OpNet began using FormAssembly in 2016 to collect student data through surveys. Over time, their use cases evolved to include everything from contract renewals and internship applications to volunteer recruitment and event coordination.
Key features they rely on today include:
- Prefill and post-submission connectors to auto-populate student data
- Auto-suggest menus for faster form filling
- Save & Resume for lengthy student applications
- E-Signature capabilities
- Custom branding to reflect OpNet’s visual identity
As usage spread across departments – including programming, career fluency partnerships, and strategic advancement – it became clear that a single-user license would no longer cut it.
Why Enterprise Made Sense
OpNet’s move from a legacy plan to an Enterprise plan was driven by several key needs:
- Expanded access: Multiple teams needed to use the platform, but the single-user setup created bottlenecks and data visibility issues.
- Improved privacy: Forms containing confidential information, such as those managed by HR, needed to be siloed appropriately.
- Enhanced security: With multi-factor authentication (MFA) becoming standard across their tools, it was crucial that FormAssembly meet this standard too.
- Form ownership and auditability: The Enterprise plan allowed OpNet to assign licenses based on role, giving better control over who could view, edit, or manage each form.

A Migration With Growing Pains
Migrating to Enterprise wasn’t without its challenges. With nearly a decade’s worth of forms and data, the OpNet team had to do some serious spring cleaning. They conducted internal surveys to prioritize active forms, archived older content, and trained staff to manage forms in the new environment.
While the planning was extensive, OpNet worked closely with FormAssembly’s implementation team to navigate the process.

A Tailored, Cross-Team Solution
Today, OpNet has 36 users on FormAssembly across departments. They’ve tiered licenses by access level and assigned data leads to maintain form governance. With features like dynamic picklists and deeper Salesforce integrations, they’re using FormAssembly more efficiently than ever.
“It makes everything cleaner. People can just log in and see only the forms they need rather than dig through 300+ forms on a shared account.”
